This option allows you to edit or create a new form based on previously added to the system. After going to "Create from survey", a window will appear. You will see a list of forms available in the system and a preview of the selected form. To edit a form, select it from the list, then click Edit. This will take you to the form creation panel. 

NOTE: the created form will be added to the system as a new one.
